Analysis Request Forms
The “Analysis Request Form”, which belongs to the relevant equipment, must be filled out completely, for each analysis sample.
The “Analysis Request Form” is also the work agrement.
By filling out the "Analysis Request Form", the customer is deemed to have read and accepted the Analysis Service Agreement that belongs to STARC.
